You can cancel or reschedule your appointment at anytime. However, you must cancel 48 hours prior to appointment time to avoid a cancellation fee of $20.

What is Medication Therapy Management (MTM)? 2017-05-16T03:40:18+00:00

MTM is personalized pharmacist -provided care that includes one-on-one consultations with a specially trained pharmacist who will:

  • Extensively review your general health and medical status

  • Help you develop a plan to achieve positive health outcomes

  • Help you manage your prescription and over-the-counter medications, and any other herbal or natural supplements you may be taking

  • Work with your other healthcare providers, like your physician, to determine if any medications need to be changed

  • help you better understand how your medications work

  • Review medications to identify, resolve, and prevent medication-related problems, including any side effects

  • Monitoring and evaluation of your response to medications, including safety and effectiveness

  • Provide training and education that will help you better understand your medications

In no way are we replacing what your doctor already does, actually, we seek to collaborate with your doctor and make recommendations that best helps our clients achieve positive health outcomes. On average, over half of americans spend only 15 minutes or less with their doctor. We believe that is just not enough time to properly empower patients to be actively engaged in their healthcare. Our pharmacists provide the care attention you deserve, and that takes much more time than just 15 minutes.

Your doctor may simply not have the time to determine whether your medications are appropriate, safe, effective, convenient, or affordable for every patient he or she sees. We extensively assess all these aspects of your therapeutic plan to provide recommendations to your doctor that best serves our clients. We are in no way replacing the care your physician provides, we just want to ensure we help him or her make the best decisions in your healthcare.

The pharmacists at your local pharmacy usually does not have time to sit and talk to you about your healthcare needs. They are so busy with other patients and filling prescriptions, they may not have the time to assess your overall health. Our services assure you are getting the full attention you deserve from a pharmacist. We offer services that provide the individualized care and attention you deserve. Our services intended to compliment what your dispensing pharmacist already does, not replace it.

Medication checkups can last between 45 minutes up to an hour and a half, it just depends on your specific health needs. However, our targeted and other wellness services usually take less time and can last anywhere between 20 and 40 minutes, but sometimes even longer.

  • Attempts to decrease the number of medications you’re taking due to unnecessary use or it just not working properly

  • Substitutions of expensive drugs for less expensive alternatives

  • Changes to your doses to either help you receive the most benefit, decrease side effects, or assure safety in certain populations (eg. elderly)

  • Eliminate any duplicate therapies

  • Recommend medications that you may benefit from but are not currently taking

If you’re taking medications your concierge pharmacist will look for:

  • Drug-drug Interactions

  • Drug-food Interactions

  • Drug-disease state interactions

  • Duplications of therapy

  • Unnecessarily low dose/high dose usage

  • Brand to generic therapeutic substitution opportunities

  • Detection of high risk medications in the elderly and other populations

  • Proper medication administration (time of day, with or without food, dosing intervals, etc)

  • Patient understanding of what each drug is being used for

  • Recommendations consistent with national evidence based guidelines are in treatment plan

  • Overuse and potential abuse issues

  • Other clinically appropriate medication related problems

Before meeting with your personal pharmacist, it is important to gather all of your medications—including prescription, over-the-counter, herbal and nutritional supplements—so they can ensure that your current drug therapy is convenient, affordable, safe and effective.

We recommend a Medication SuitCASE up at least 3 times a year. Or, if changes occur to your healthcare, such as a new diagnosis of a chronic illness or a recent hospital visit. But, there is no limit to the amount of medication checkups you can receive. We also provide many other services so our clients can receive an ongoing assessment of their health status and medications.

Although we are specially trained pharmacists, MedVize aims to only focus on making sure you are empowered to live a happier, healthy life. We only provide consulting services and do not dispense any medications. You are encouraged to continue to use your local community pharmacists to receive your prescriptions.

You can contact us if you have a simple health related question and we will try to respond within 48 hours. However, if we feel it is more complex, we may recommend one of our targeted services to ensure you get the best answer possible to help you live a healthier, happier life. But, sometimes a simple question may be best left up to your local community pharmacist.

Our services are intended to provide an in depth review of your personal health needs so we can better provide education and advice to best fit your individual situation. And this usually takes more than just the average 2 minutes you may spend with your local community pharmacist.
